Ways to Apply for Himmat Card Registration
The Punjab government has introduced two methods of registration for Himmat cards for the convenience of disabled persons. One procedure is online procedure in which you need an internet connection while the other procedure is offline in which you have to register yourself by visiting the center established by the Punjab government.

Registration Process Via PWD App
The online registration procedure for the Himmat card is the registration procedure through the PWD App for which you need an internet connection. You can get a Himmat Card through the PWD App by following the steps mentioned below:
- Install the PWD App in your system.
- Create your new account with the help of a CNIC number, phone number, and a strong password.
- Log in to the account using your National Identity Card number and password.
- On successful login you will be shown some options from these options choose the “Doctor Appointment” option and get your appointment from your doctor.
- In the appointment, the doctor will check the condition of your disability and if he considers you disabled, he will provide you with a disability certificate from the Government of Punjab.
- After obtaining the certificate, click on the “Registration Form” option.
- You will be provided with a form, in this form, you have to provide your personal information, you have to provide information about your disability, and you have to provide your education and income information.
- You will be asked for a Disability Certificate from the Punjab Government. Provide your disability certificate, upload your identity documents, and complete your registration process by clicking on the “Submit” button.

Registration Through DHQ Hospital
If you do not have an internet facility, you can also register offline with Himmat Card. In the offline procedure, you have to visit the nearest DHQ Hospital. You can confirm your registration through DHQ Hospital by following the steps given below:
- Collect the documents required for registration and visit the nearest DHQ Hospital.
- Book your appointment with the doctor there and get the disability certificate from the doctor.
- Contact the representative at the reception of DHQ Hospital for issuance of Himmat Card.
- The representative will give you a form in which you have to provide your personal information.
- Attach your documents including your disability certificate and CNIC documents to the form and submit it to the representative.
- After submitting the form and documents you will be successfully registered in Himmat Card.
